How it works

Health Maintenance Organization, otherwise known as an HMO, is a plan that requires members to select a Personal Care Physician or PCP. The PCP is the member’s go-to doctor and first-line partner for better health who will coordinate all of their health care needs. If the need arises, specialty care is simple with our paperless referral system.

HMO service area & network

The HAP HMO service area includes the following nine counties: Genesee, Lapeer, Livingston, Macomb, Monroe, Oakland, St. Clair, Washtenaw and Wayne. In addition, members are covered for emergency and urgent care whenever they travel outside of our service area - worldwide.

The HAP HMO network is a subset of the statewide network and encompasses the nine-county service area. Each HAP PCP belongs to a network of doctors, medical centers and hospitals. When a member selects a PCP they are also selecting a network. Most times their PCP will refer their patient for specialty and hospital care within their network. However, most of our PCPs and specialists are part of a larger system where they can refer the member to any specialist or hospital within our service area for greater flexibility.


Health Engagement program

HAP’s Health Engagement program is a two-tier HMO plan design. Employer groups with 51 or more employees can choose a plan designed with incentives that encourage members to adopt a healthy lifestyle. Both programs measure key health factors that can be controlled through lifestyle choice and positively impact employee health and performance.
